Alex: Good afternoon, you're through to THT Direct. How can I help?

Caller: Oh hi, yeah. I’m not sure if this is the right number actually. I’ve got a question about PrEP.

Alex: Absolutely, that’s what we’re here for. What’s going on?

Caller: So I’ve been on PrEP for about three months through the clinic. I thought I understood how to take it but I’ve been reading stuff online and now… I’ve confused myself.

Alex: That happens more than you’d think. There’s a lot of information out there. What specifically is confusing you?

Caller: I’m on daily dosing. I’ve been taking it every day. But last week I had a couple of nights out and took it later than usual, a few hours late. Then I read something that said if you miss the timing it might not work as well, and I got panicked because I wasn’t being as careful as I should have been.

Alex: I’m glad you called. When you say later than usual, was it a couple of hours or did you miss a whole day?

Caller: No, I didn’t miss a day. Both times I took it the same day, just four or five hours later.

Alex: Right. For daily PrEP, the guidance is taking it at roughly the same time matters, but a few hours later is not the same as missing a dose. You still took it. The key thing is consistent levels in your system, and a few hours’ variation does not undo that. Missing the odd dose doesn’t affect protection. I can refer you to a PrEP guide if helpful.

Caller: Okay. (exhales) That’s a relief. The thing I read made it sound like even being slightly off could be a problem. A guide would be helpful.

Alex: I know. A lot of what you see is not nuanced. Timing matters more with on demand dosing. You’re on daily, which is more forgiving of small variations, and even the odd missed dose if you’ve been consistent.

Caller: Okay…So I’m probably fine?

Alex: Based on what you’ve described, yes. You’ve been taking it consistently for three months, and a few hours either way doesn’t undermine protection. I’m not a clinician, so if you have ongoing concerns it’s worth contacting your clinic too.

Caller: That makes sense. I have a good relationship with the clinic. I think I just spiralled reading forums and didn’t want to bother them.

Alex: That’s so common. Forums late at night can be one of the worst places for health information. Is there anything else worrying you?

Caller: (pause) I guess I sometimes wonder if I’m doing everything right. It’s still new to me and I don’t feel like I can ask the clinic every little thing.

Alex: Please don’t feel that way. The clinic would rather you asked than sat on something. That’s part of why we exist. You can call us with things that feel too small. There’s no question too basic here.

Caller: That’s reassuring. I didn’t know this service existed until tonight.

Alex: A lot of people don’t. But you found us. Are you feeling clearer about the PrEP side?

Caller: Yeah, a lot clearer. I just needed someone to talk me through it.

Alex: Exactly. If you’re ever in that situation again, just call.
