Young person referral form (Oxfordshire)

Please do not submit a referral using this form until you have read the 1:1 Education Programme’s criteria (this will open a PDF in a new window).

All forms must be completed in full. Any information missing from the required areas of the form may result in an automatic decline. Any referrals submitted to individual team members using the old referral forms will be asked to re-submit to the website. If the staff member submitting the referral is not the organisational safeguarding lead, please ensure you seek their approval beforehand.

Please note we do not work with young people currently receiving support from another service, or who are the subject of a current, open police investigation.
